Specialist support Coordination to support participants who are accessing services across multiple areas (for example: NDIS, as well as mental health, justice and/or child protection systems). Kindly Care Australia Support Coordinators providing SSC have qualifications, skills and experience in supporting participants with complex needs. Specialist Support Coordinators work with participants and supporters to identify needs, develop meaningful goals, and ensure appropriate supports are in place to manage challenges and risks to participant safety and well being.
Our Specialist Support Coordinators provide extra support when a participant has complex needs or higher levels of risk. They work closely with the participant, their support team and other services to help manage challenges, coordinate the right supports and improve safety and wellbeing. The support is time-limited and focuses on addressing barriers, reducing complexity in the support environment, and helping the participant connect with services and build capacity and resilience. Where needed, this may include developing a practical support plan that outlines agreed actions, strategies and services to support the participant. The plan may be implemented with the assistance of disability and mental health support workers. It is not a Behaviour Support Plan or clinical treatment plan.
