Support Coordination helps participants understand and use their NDIS plan, connect with providers, coordinate supports, and work towards their goals and greater independence.

Specialist Support Coordination is for participants with more complex needs or circumstances who require a higher level of support to coordinate services, address barriers, and manage complex situations.

Our Support Coordinators & Specialist Support Coordinators can:

• Help you understand your NDIS plan
• Identify and negotiate with suitable service providers
• Assist with the necessary documentation and paperwork
• Support your choices
• Assist with coordinating appropriate services and escalation during times of crisis, where required. Support Coordinators do not replace emergency, medical or clinical crisis services.
• Help you navigate service systems, coordinate providers and address barriers to accessing the supports you need
• Our goal is to find you the support you need to live the life you choose build your confidence and capacity to understand and manage your supports as independently as possible.
