Specialist support Coordination to support participants who are accessing services across multiple areas (for example: NDIS, as well as mental health or justice or child protection). Kindly Care Australia Support Coordinators providing SSC have qualifications, skills and experience in support participants with complex needs. Specialist Support Coordinators work with participants and supporters to identify needs, develop meaningful goals, and ensure supports are in plan to manage challenges and risks to participant safety and well being.

Our Specialist Support Coordinators work within an NDIS specialist framework, necessitated by high-level risks present in the participant’s current situation. The support is time-limited and focuses on addressing barriers and reducing complexity in the support environment while assisting the participant to connect with services and build capacity and resilience. It may also involve the development of an intervention plan which will be put in place and supported by Disability or Mental Health support workers.
