Life Transitions & Support Planning
Assistance with Life Stage Transition provides short- and long-term support to strengthen participants’ abilities, independence and choices during significant changes in their lives. Transitions such as moving out of home, starting university, TAFE or other study, or entering employment can be exciting but may also bring emotional and practical challenges. For people with disability, these changes can involve additional barriers, new environments and unfamiliar routines.

Kindly Care Australia works alongside participants to plan and prepare for these changes, identify potential challenges and put practical supports in place. Our aim is to reduce barriers, build confidence and help participants develop the skills they need to manage change and adjust to new environments. We provide personalised support throughout the transition, helping participants make informed choices, maintain their independence and work towards a positive and successful outcome.
